SOUTHSIDE PEDIATRICS IMMUNIZATION AND VISIT SCHEDULE

BIRTH: (at hospital): Hepatitis B #1

1 MONTH: Hepatitis B #2

2 MONTH: Pentacel #1, Prevnar #1, Rotavirus #1

4 MONTH: Pentacel #2, Prevnar #2, Rotavirus #2

6 MONTH: Pentacel #3, Prevnar #3, Rotavirus #3, Hepatitis B #3

9 MONTHS: no shots if up to date

12 MONTHS: Prevnar #4, Hepatitis A #1, MMR or Varicella #1, lead level fingerstick

15 MONTHS: Pentacel #4, Varicella or MMR #1

18 MONTHS: Hepatitis A #2

2 YEAR: lead level fingerstick

Then YEARLY: well checks to monitor growth, development, health

4-5 YEAR: Quadracel, Proquad, vision screen, urine screen

11 YEAR: Tdap #1, Menactra #1, urine screen, * may start HPV series

16 YEAR: Tdap#2, Menactra #2, Men B#1 (#2 dose at least one month later)

Annual FLU SHOTS recommended for all patients 6 months and older (usually available September through February). The first year a patient less than 8 years old receives a flu vaccine, they require 2 doses at least one month apart.

*HPV: 2 doses 6 months apart if started anytime before pt turns 15yrs, otherwise it is a 3 dose series

Rotavirus vaccine is an oral liquid

Pentacel= Dtap, Hib, Polio

Proquad= MMR, Varicella

Quadracel= Dtap, Polio

Menactra= meningitis strains A,C,Y,W

Men B= meningitis strain B
