Birth: Hep B #1
*October-March, if no maternal dose 14 or more days before delivery: RSV IG
(within 1 week of delivery or immed prior to season if under 1yr)

2 month: Vaxelis #1 (DTaP, Polio, HIB, HepB), Prevnar #1, Rotavirus #1

4 month: Vaxelis #2, Prevnar #2, Rotavirus #2

At least 6 months: Vaxelis #3, Prevnar #3, Rotavirus #3
*at 6 months and older, eligible for seasonal flu and COVID vaccines
*October-March, if high risk 8-19mo : RSV IG#2

9 month: no vaccines due if up to date

At least 12 months: Prevnar #4, MMR #1, Hep A #1

15 month: Pentacel (DTaP#4, Polio#4, HIB#4), Varicella #1

18 month: Hep A #2 (6 months after #1)

4-5 year: Proquad (MMR #2,Varicella #2), Quadracel (DTaP #5, Polio #5)

At least 11 year: Tdap #1, Menactra #1 (Meningitis strains A,C,Y,W)
*eligible for HPV vaccine

At least 16 year: Tdap #2, Menactra #2
*eligible for Men B (meningitis strain B) vaccine- required for dorms/barracks
